To make an entry people can reply to but no one can see what people respond, would I make the "screen comments" line = "all comments." and then just not approve any?

I believe that's the case. As [livejournal.com profile] hrafn found out recentely, though, responding to those comments automatically unscreens them.

I've known some people to workaround this by rescreening the original comments immediately after responding. So, they are potentially visible during a brief period, but still essentially private.

Yes, yes, and yes. I do that, re-screen them immediately if I respond to them. They're only visible for a second or two.
